When a concrete company was supplying the basement of a construction site, due to the unfamiliar business level of the batching plant operator the tester wrote the concrete mixing design and handed it over to the operator for input. He went to the concrete laboratory to play mobile games, and the operator mistakenly input the cement. The fly ash was used as cement. The problem was discovered when the fly ash in silo ran out, and more than 500 cubic meters of concrete were already supplied. As a result, the steel bars were cut off, the concrete was dug up, and the cost of compensation for the construction site plus the loss of concrete amounted to more than 2 million yuan.

Precaution

(1) Strengthen the professional knowledge training of testers and operators, and be familiar with the names of the materials stored in each silo and the computer before starting work;

(2) After the operator's computer inputs the ratio, he must sign the review sheet after being reviewed and confirmed by the tester before the concrete production can be organized. In addition, for the production of the first batch of concrete, the tester must be present to do a good job of appraisal;

(3) The company must formulate a set of strict management system, in accordance with the quality management system and operating instructions.
